The Office of Child Care Regulation regulates different types of child care providers and can only investigate complaint allegations relevant to the governing statutes and/or rules for child care services. In the Complaint Narrative section please include as much detailed information as possible. This includes a description of the incident as well as answers to the basics questions like who was involved, what occurred or was observed, when did the incident happen, where did the incident occur, etc.

You may submit your complaint anonymously, however if there is not enough information entered into the complaint narrative, and we cannot contact the complainant for more information, the complaint may not be investigated.
